阿肯（黎駿 飾）帶著女友玲玲（徐玲玲 飾）以及一干好友前往一座地處偏遠的荒島，準備在那里開一場熱鬧而又淋漓盡致的派對。然而抵達小島后，一行人才發(fā)現，原來他們并不是島上唯一的一群游客。咸濕、吹水和山豬（黃秋生 飾）三名古惑仔也來到了小島上，他們來這里是為了探訪在這兒隱居的黑幫老大。
黑道的出現讓阿肯一行人感受到了不安全，然而，他們卻不是真正的問題所在。咸濕和山豬先后被殺，剩下來的吹水找到了阿肯向他們尋求幫助，這意味著，在這座島嶼上，隱藏著一批嗜血而又兇殘的殺人狂魔，更糟糕的是，他們的真實身份，是一群麻風病人。
